BPJS Health in Indonesia currently implements an inpatient class system which is divided into three levels: class 1, class 2, and class 3, with differences in facilities and fees paid by participants. However, Presidential Regulation Number 59 of 2024 initiated the replacement of this system with the Standard Inpatient Class (KRIS) starting June 30 2025. KRIS aims to provide equal inpatient services regardless of the amount of contributions, ensuring that all participants receive the same quality of service without discrimination. KRIS standards include 12 facility criteria, including air ventilation, lighting, and ensuite bathrooms, to increase patient comfort and privacy. The main challenges in implementing KRIS include the need for large investments to improve hospital facilities and infrastructure, especially in remote areas, as well as the BPJS Health budget deficit which will reach IDR 7,9 trillion in 2023. The government proposes the use of Special Allocation Funds (DAK) to support improving facilities hospitals, with a total national health budget of IDR 186,4 trillion in 2024. The effectiveness of outreach and education regarding KRIS is also important to ensure public understanding and support for this change, which aims to improve the accessibility and quality of health services in Indonesia.
